Northwest Community Care Network (NCCN) is one of 14 independent networks that make up Community Care of North Carolina (CCNC).  Established in 1998, CCNC is an award-winning public-private partnership between the state of North Carolina and local and regional partners, bringing coordinated care to all 100 NC counties.

Northwest Community Care Network (NCCN) supports a patient-centered approach to health care. We partner with doctors, pharmacists, hospitals, health departments, social services, behavioral health and community organizations.  Together, we offer coordinated care through what’s known as the Medical Home model.  This model links patients to a primary care doctor who leads a team of professionals offering the right services at the right time in the right setting.  NCCN serves Davidson, Davie, Forsyth, Stokes, Surry, Wilkes and Yadkin Counties.
